Rhinoplasty Before and After: How to Read the Photos

✔  QUICK ANSWER

Most rhinoplasty before-and-after photographs you find online are taken far too early to mean anything. The splint comes off at day 7, the nose still carries obvious swelling at three months, and the true final shape — particularly the tip — settles at around twelve months. A pair of photographs is only informative if you know when the second one was taken and both were shot the same way. Learn to read them properly and you will judge surgeons on evidence rather than on photography.

Why Timing Changes Everything

StageWhat the nose looks likeIs it a fair “after”?
Day 7Splint off. Swollen, wide, tip thick. No — this is oedema
Weeks 2–6Visible swelling drops; shape emergingNo
3 monthsBridge looks clean; tip still fullNo — but this is when most photos are taken
6 monthsClose, tip continuing to refineGetting there
12 monthsTip defined; deep swelling resolvedYes
18 monthsFinal for thick skin and revisionsYes

The reason the three-month photograph is so tempting is that the bridge is already clean by then — the dorsum settles fast. The tip does not, and the tip is what most patients actually care about. The full sequence is in the week-by-week recovery guide.

What to Check in Any Before-and-After Photo

  • The same angle in both. A few degrees of head rotation hides a dorsal hump or manufactures a refined tip. Look for a true profile with the eye, ear and shoulder in the same relationship in each shot.
  • The same lighting. Harsh overhead light throws a shadow that exaggerates a hump; soft frontal light flattens it. If the “before” is dim and clinical and the “after” is bright and warm, you are looking at photography.
  • The same head tilt. Chin down lengthens the nose. Chin up shortens it. This one alone can fake a result.
  • The same distance and lens. A close phone shot enlarges whatever is nearest the lens — your nose. A “before” taken at arm’s length and an “after” taken from two metres are not comparable.
  • No makeup change. Contouring alone visibly narrows a nose.
  • A stated interval. “After” with no date is not evidence.
  • The base view. Nostril symmetry and tip definition show from below and nowhere else — which is precisely why it is the view most often missing.

Red Flags

  • Only frontal photographs, never a true profile or base view
  • Every result looks like the same nose. A signature being applied to every face rather than a plan built for each one. This is the most important red flag on the list and the most commonly missed.
  • Different image quality, crop or resolution between the two photographs
  • Results at three months presented as final
  • No thick-skinned patients anywhere in the gallery. Thick skin is common; a gallery containing only thin-skinned results is a selection, not a portfolio.
  • Stock-looking models, or images that also appear on other clinics’ websites
  • No revision cases shown by a surgeon claiming revision expertise

Simulations, Morphs and Filters

Digital simulation is a communication tool, not a promise. It is genuinely useful for agreeing direction — how much hump comes down, how much rotation — and it is worth doing at consultation. What it cannot do is predict how your skin and cartilage will heal. No software models scar contracture or skin thickness.

Social media filters are a separate and more serious problem. Filters slim the nose in ways surgery cannot replicate, and patients increasingly arrive asking for a face that only exists in an app. A surgeon who agrees to reproduce a filtered photograph is agreeing to something anatomy will not deliver. Bring unfiltered pictures of yourself and of results you like — the conversation is much more useful.

Why There Is No Public Gallery on This Site

Two reasons, and both deserve stating plainly.

Consent. Every published photograph needs explicit, informed and revocable consent. Many patients are delighted with their result and still do not want their face used in marketing — and that decision belongs to them, not to the clinic.

Regulation. Health-advertising rules in Turkey place tight limits on the promotional use of patient before-and-after imagery. Clinics publishing large public galleries are not necessarily operating within them.

What happens instead: during consultation, cases with anatomy comparable to yours — similar skin thickness, similar dorsal profile, similar tip support — are reviewed with you and the reasoning explained. That is considerably more useful than scrolling strangers, because the only result that predicts yours is a nose that started like yours.

How to Photograph Your Own Nose Properly

Before any consultation, take these yourself — they make a remote assessment far more accurate:

  • Front, camera at eye level, neutral expression, hair back
  • True profile, both sides — not three-quarter
  • Base view, head tilted back, showing the nostrils and columella
  • Smiling profile — this reveals a tip that drops on smiling, which is easy to miss otherwise
  • From about a metre away, in even indoor daylight, no filter, no makeup on the nose

What You Can Realistically Expect

Rhinoplasty changes bone, cartilage and soft tissue. It does not change your skin thickness, your healing biology or your facial proportions — and those three set the ceiling on any result. Thick skin refines more slowly and less sharply than thin skin; that is anatomy, not surgical skill.

An honest surgeon tells you where your ceiling is before you book. If you are weighing surgery at all, start with how to vet a surgeon and the guide to open versus closed technique, and read how to tell a real problem from swelling if you are unhappy with previous surgery.

Frequently Asked Questions

When is the final rhinoplasty result visible?

Around twelve months for most people, and up to eighteen for thick skin or revision cases. The bridge settles within weeks; the tip is always last.

Why do rhinoplasty before-and-after photos look so different from real results?

Usually because of timing and photography. Most published “after” images are taken at about three months, when the bridge looks finished and the tip does not, and are often shot in more flattering light and at a different angle from the “before”.

What should I look for in before-and-after photos?

The same angle, lighting, distance and head tilt in both images, a stated time interval of twelve months or more, and a base view. Then ask whether the patient still looks like themselves.

Why does every result in some galleries look the same?

Because one technique is being applied to every nose rather than a plan built around each face. A nose should be planned around the face it belongs to.

Can I see before-and-after cases before deciding?

Yes — in consultation, where cases with anatomy comparable to yours can be reviewed and the reasoning explained. Start by sending photographs on WhatsApp.

Do computer simulations predict my result?

They communicate direction, not outcome. No software accounts for how your skin and cartilage will heal, and none models scar contracture or skin thickness.

Can surgery give me the nose from a filtered photo?

No. Filters reshape faces in ways anatomy does not allow. Bring unfiltered photographs — of yourself and of results you like — and the conversation becomes a realistic one.
